I had just arrived in Vancouver to lead a research project

麻薬を注射している人を 初めて目にしたときの事が目に浮かびます

in HIV prevention in the infamous Downtown East Side.

It was in the lobby of the Portland Hotel,

a supportive housing project that gave rooms

to the most marginalized people in the city,

the so-called "difficult to house."

I'll never forget the young woman standing on the stairs

repeatedly jabbing herself with a needle, and screaming,

In response to the desperate state of affairs, the drug use,

the poverty, the violence, the soaring rates of HIV,

Vancouver declared a public health emergency in 1997.

This opened the door to expanding harm reduction services,

distributing more needles,

increasing access to methadone,

and, finally, opening a supervised injection site.

But today, 20 years later,

harm reduction is still viewed as some sort of radical concept.

In some places, it's still illegal to carry a clean needle.

ハーム・リダクションはかなり 急進的な考えだと思われています

Drug users are far more likely to be arrested

ある地域では 清潔な注射針の所持さえ いまだに犯罪となります

than to be offered methadone therapy.

薬物使用者はメサドン療法を 提供されるよりも

Recent proposals for supervised injection sites

逮捕されてしまう事のほうが はるかに多いでしょう

in cities like Seattle, Baltimore and New York

最近では 薬物使用室の設置が

have been met with stiff opposition:

opposition that goes against everything we know about addiction.

強硬な反対にあっています

Why is that?

薬物依存について私たちが知っている すべての事への反対です

Why are we still stuck on the idea

なぜでしょうか？

that the only option is to stop using -- that any drug use will not be tolerated?

なぜ私たちは こう思い込むのでしょうか？

Why do we ignore countless personal stories

「薬物使用を止める事が唯一の方法だ どんな薬物使用も許容すべきでない」

and overwhelming scientific evidence

なぜ一人ひとりの個人的な事情や

that harm reduction works?

ハーム・リダクションの有効性を示す

Critics say that harm reduction doesn't stop people

優れた科学的根拠を 無視するのでしょうか？

from using illegal drugs.

ハーム・リダクションでは違法薬物の使用は なくならないという批判があります

Well, actually, that is the whole point.

まさにそこがポイントです

After every criminal and societal sanction

これまでどんな刑罰や 社会的な制裁を与えても

that we can come up with,

薬物使用はなくなりませんでしたし たくさんの人が亡くなってきました

people still use drugs, and far too many die.

私たちが彼らを見限っている という批判もあります

Critics also say that we are giving up on people

依存の治療や回復に 注力していないというのです

by not focusing our attention on treatment and recovery.

実際には 全く逆です

In fact, it is just the opposite.

私たちは彼らを見捨てていません

We are not giving up on people.

依存から回復させようとするなら

We know that if recovery is ever going to happen

まず彼らの命を守らなければなりません

we must keep people alive.

清潔な注射針を提供したり 安全な薬物使用室を用意するのは

Offering someone a clean needle or a safe place to inject

治療や回復の第一歩なのです

is the first step to treatment and recovery.

こういう批判もあります

Critics also claim that harm reduction

ハーム・リダクションは子供たちに 誤ったメッセージを送るというものです

gives the wrong message to our children about drug users.

しかし私が最近会った 薬物依存者は子供でした

The last time I looked, these drug users are our children.

ハーム・リダクションの送るメッセージは 薬物には害があるとしたうえで

The message of harm reduction is that while drugs can hurt you,

薬物依存者には救いの手を 差し伸べる必要があるという事です

we still must reach out to people who are addicted.

注射針の提供は 薬物使用を 宣伝するものではありません

A needle exchange is not an advertisement for drug use.

メサドン療法も薬物使用室も同様です

Neither is a methadone clinic or a supervised injection site.

気にかけるべきは 人々が病み傷ついている事なのです

What you see there are people sick and hurting,

断じて薬物使用を 是認している訳ではありません

hardly an endorsement for drug use.

薬物使用室を例に お話ししましょう

Let's take supervised injection sites, for example.

おそらく今まででもっとも誤解を 受けている医療的介入です

Probably the most misunderstood health intervention ever.

私たちが主張しているのは

All we are saying is that allowing people

清潔で乾いた場所で 未使用の注射針を使い

to inject in a clean, dry space with fresh needles,

見守る人がいる状態で注射させる方が

surrounded by people who care

薄汚い路地で注射するより ずっとマシだという事です

is a lot better than injecting in a dingy alley,

汚染された注射針を使い回し 警官から隠れながら注射するより

sharing contaminated needles and hiding out from police.

誰にとってもマシなのです

It's better for everybody.

バンクーバーで最初の薬物使用室は キャロル通り327にありました

The first supervised injection site in Vancouver was at 327 Carol Street,

コンクリート床の狭い部屋には 数脚のイスと清潔な注射針が１箱ありました

a narrow room with a concrete floor, a few chairs and a box of clean needles.

しばしば警官に封鎖されましたが

The police would often lock it down,

どういうわけか いつも不思議と再開していました

but somehow it always mysteriously reopened,

often with the aid of a crowbar.

I would go down there some evenings

to provide medical care for people who were injecting drugs.

I was always struck with the commitment and compassion

of the people who operated and used the site.

批判なく 嫌がらせもなく なんの心配もなく

No judgment, no hassles, no fear,

多くの深遠な会話が 交わされていました

lots of profound conversation.

想像もつかないトラウマや 肉体的な痛み

I learned that despite unimaginable trauma,

そして精神的な病にもかかわらず

physical pain and mental illness,

そこにいた全員が 状況が良くなると 考えている事を知りました

that everyone there thought that things would get better.

ほとんどの人が いつかはみんな一緒に 薬物をやめるだろうと確信を持っていたのです

Most were convinced that, someday, they'd stop using drugs altogether.

その部屋は 北アメリカで 最初に政府に承認された

That room was the forerunner to North America's

先駆けとなる薬物使用室で 「INSITE」と名付けられました

first government-sanctioned supervised injection site, called INSITE.

3年間の研究プロジェクトとして 2003年9月に開設されました

It opened in September of 2003 as a three-year research project.

保守的な政府は 研究終了後 そこを閉じるつもりでした

The conservative government was intent on closing it down at the end of the study.

8年の後 INSITEの閉鎖を めぐるせめぎ合いは

After eight years, the battle to close INSITE

カナダ最高裁で争われるまでに いたりました

went all the way up to Canada's Supreme Court.

カナダ政府と争ったのは

It pitted the government of Canada

長年にわたり 薬物を使用した過去があり

against two people with a long history of drug use

INSITEの利点を 身をもって知る２人

裁判所は９対０の評決をもって INSITEの存続を言い渡しました

The court ruled in favor of keeping INSITE open by nine to zero.

The justices were scathing in their response to the government's case.

紹介します

And I quote:

ハーム・リダクションにとって 希望の瞬間でした

This was a hopeful moment for harm reduction.

しかしながら この最高裁からの 力強いメッセージをもってしても

Yet, despite this strong message from the Supreme Court,

ほんのつい最近まで

it was, until very recently,

カナダにおいて新しい施設を 開設する事はできませんでした

impossible to open up any new sites in Canada.

2016年12月に 興味深い事が起こりました

There was one interesting thing that happened in December of 2016,

薬物の過剰摂取による 死亡者が増加していたため

when due to the overdose crisis,

ブリティシュ・コロンビア州政府が 過剰摂取対策施設の開設を認めたのです

the government of British Columbia allowed the opening of overdose prevention sites.

実質的には カナダ連邦の 認可プロセスを無視して

Essentially ignoring the federal approval process,

地域団体は 事実上不法の薬物使用室を

community groups opened up about 22 of these de facto illegal

州内の22カ所に開設しました

supervised injection sites across the province.

ほとんど一夜にして

Virtually overnight,

何千人もが管理下で薬物を 使用できるようになりました

thousands of people could use drugs under supervision.

何百もの過剰摂取者がナロキソン投与で回復し 誰も死ぬ事はありませんでした

Hundreds of overdoses were reversed by Naloxone, and nobody died.

まさにこれこそが この14年間 INSITEで起こってきた事なのです

In fact, this is what's happened at INSITE over the last 14 years:

INSITEで違法薬物を注射した人は ７万５千人にのぼり

75,000 different individuals have injected illegal drugs

回数は 延べにして350万回を超えます

more than three and a half million times,

そして 亡くなった人は誰もいません

and not one person has died.

誰一人として いまだかつて INSITEで亡くなっていないのです

Nobody has ever died at INSITE.

もうお判りでしょう

So there you have it.

私たちには 注射針の提供や メサドンや薬物使用室による

We have scientific evidence and successes from needle exchanges

科学的な根拠があり 成功も収めました

methadone and supervised injection sites.

これらは皆の共通の認識です 薬物使用に寄り添う事によって

These are common-sense, compassionate approaches to drug use

健康状態を改善し 結びつきを強め

that improve health, bring connection

そして苦悩や死を 大幅に減らせるのです

and greatly reduce suffering and death.

では なぜハーム・リダクションは 普及しないのでしょう？

So why haven't harm reduction programs taken off?

なぜ 私たちはいまだに薬物使用を 刑罰の対象として考えるのでしょう？

Why do we still think that drug use is law enforcement issue?

私たちの薬物や薬物使用者に対する 侮蔑には根深いものがあります

Our disdain for drugs and drug users goes very deep.

私たちは 薬物の恐ろしさに関するイメージや

We are bombarded with images and media stories

メディアの記事にさらされています

about the horrible impacts of drugs.

薬物に関わる人すべてに 汚名を着せているのです

We have stigmatized entire communities.

軍が行う麻薬業者の撲滅活動を称賛し

We applaud military-inspired operations that bring down drug dealers.

薬物使用だけの罪で 捕らわれた人を収容するための

And we appear unfazed by building more jails

刑務所の増設を当然のように受け止めます

to incarcerate people whose only crime is using drugs.

何百万人にもおよぶ人が 収監、暴力、貧困といった

Virtually millions of people are caught up

救いようのないサイクルから 抜け出せずにいます

in a hopeless cycle of incarceration, violence and poverty

それは薬物そのものではなく 薬物の法律によって引き起こされてきたのです

that has been created by our drug laws and not the drugs themselves.

どうしたら薬物使用者にも 気配りや 支援を受け 人生を謳歌する権利があると

How do I explain to people that drug users deserve care and support

人々に納得してもらえるでしょうか？

and the freedom to live their lives

世間は薬物について 銃や手錠や牢獄を 思い浮かべるばかりなのに

when all we see are images of guns and handcuffs and jail cells?

はっきりさせましょう

Let's be clear:

刑罰化は 汚名を制度化する手段にすぎません

criminalization is just a way to institutionalize stigma.

薬物を非合法化しても 薬物の使用を防ぐ事はできないのです

Making drugs illegal does nothing to stop people from using them.

私たちが思考停止に陥るのは

Our paralysis to see things differently

薬物に関する全く誤った 認識のせいでもあります

is also based on an entirely false narrative about drug use.

私たちは信じさせられてきました

We have been led to believe that drug users

薬物使用者は責任感がなく ただハイになりたいだけで

are irresponsible people who just want to get high,

自分のしでかした失敗のせいで

and then through their own personal failings

犯罪や貧困に陥ってしまい

spiral down into a life of crime and poverty,

仕事も家族も失い 最後には自分の命まで 失ってしまうような人間だと

losing their jobs, their families and, ultimately, their lives.

現実には ほとんどの薬物使用者には 物語があります

In reality, most drug users have a story,

子供のときのトラウマや 性的虐待や精神疾患

whether it's childhood trauma, sexual abuse, mental illness

もしくは個人的な悲劇など

or a personal tragedy.

薬物は痛みを感じなくするために 使われるのです

The drugs are used to numb the pain.

強いトラウマを抱える人々と接する場合は それを理解していなければなりません

We must understand that as we approach people with so much trauma.

大事なのは 私たちの薬物対策は 社会的正義の問題だという事です

At its core, our drug policies are really a social justice issue.

メディアは過剰摂取事故に注目します プリンスやマイケル・ジャクソンのときのように

While the media may focus on overdose deaths like Prince and Michael Jackson,

しかし苦しんでいる人の大半は

the majority of the suffering

社会から取り残されそうな生活を送る人々

happens to people who are living on the margins,

貧乏だったり より所のない人々です

the poor and the dispossessed.

彼らは選挙に行きません しばしば孤独です

They don't vote; they are often alone.

社会的に打ち捨てられた存在なのです

They are society's disposable people.

医療を受けるにしても 薬物使用は強い偏見に見舞われます

Even within health care, drug use is highly stigmatized.

薬物使用者は医療を受ける事を避けます

People using drugs avoid the health care system.

医療を受けるために医者に行ったり 入院したりすると

They know that once engaged in clinical care

ぞんざいに扱われると 分かっているからです

or admitted to hospital, they will be treated poorly.

また ヘロイン、コカイン、覚せい剤が

And their supply line, be it heroin, cocaine or crystal meth

入手できなくなるからです

will be interrupted.

そのうえ 彼らは容赦ない 質問の嵐にさらされるわけですが

On top of that, they will be asked a barrage of questions

それは彼らの失ったものや屈辱を 明らかにするだけです

that only serve to expose their losses and shame.

「使っている薬物は？」

"What drugs do you use?"

「ホームレスになってどのくらい？」

"How long have you been living on the street?"

「子供はどこにいる？」

"Where are your children?"

「前回はいつ刑務所に？」

"When were you last in jail?"

要するに こう言いたいのです 「一体なぜ薬物をやめないんだ？」

Essentially: "Why the hell don't you stop using drugs?"

実際のところ 現在の薬物使用に対する 医療的アプローチは全く間違っています

In fact, our entire medical approach to drug use is upside down.

どういうわけか

For some reason,

人々は薬物依存を治療するには薬物を断つ事が 一番良い方法だと決めつけています

we have decided that abstinence is the best way to treat this.

運が良ければ薬物離脱プログラムを 受けられるでしょう

If you're lucky enough, you may get into a detox program.

サボキソンやメサドンが 入手しやすい環境に住んでいれば

If you live in a community with Suboxone or methadone,

薬物代替プログラムを 受けられるかもしれません

you may get on a substitution program.

しかし彼らが生き延びるのに どうしても必要な

Hardly ever would we offer people what they desperately need to survive:

安全なオピオイドの処方は めったに得られません

a safe prescription for opioids.

いきなり薬物を断たせようとするのは 糖尿病になったばかりの患者に砂糖を断たせたり

Starting with abstinence is like asking a new diabetic to quit sugar

ひどい喘息の患者にマラソンを走らせたり

or a severe asthmatic to start running marathons

鬱の人にただ幸せになれと 指示するようなものです

or a depressed person to just be happy.

他の病気の治療なら

For any other medical condition,

一番極端な方法から 始めはしないでしょう

we would never start with the most extreme option.

それなのに なぜ依存症のような複雑な疾患に

What makes us think that strategy

その極端な方法が効果的だと 考えてしまうのでしょうか？

would work for something as complex as addiction?

意図しない過剰摂取の問題は 昔からありますが

While unintentional overdoses are not new,

現在の危機的状況はこれまでとは 比べものになりません

the scale of the current crisis is unprecedented.

疾病管理センター（CDC）の推計によると

The Center for Disease Control estimated

2016年には 6万4千人のアメリカ人が 薬物の過剰摂取で亡くなっています

that 64,000 Americans died of a drug overdose in 2016,

Drug-related mortality is now the leading cause of death

among men and women between 20 and 50 years old in North America

考えてみてください

Think about that.

どうして そしてなぜ今 こんな状況に陥ってしまったのでしょう？

How did we get to this point, and why now?

実はこれはオピオイドが 引き起こした災いなのです

There is a kind of perfect storm around opioids.

オキシコンチン、パーコセット、 ディラウディッド等の薬物が

Drugs like Oxycontin, Percocet and Dilaudid

あらゆる種類の痛みを抑えるため 数十年にわたり野放しに流通されてきました

have been liberally distributed for decades for all kinds of pain.

推定200万人のアメリカ人が オピオイドを日常的に使用しており

It is estimated that two million Americans are daily opioid users,

昨年１年間で ６千万人以上のアメリカ人が

and over 60 million people

オピオイドを少なくとも１回は 処方されています

received at least one prescription for opioids last year.

This massive dump of prescription drugs into communities

医者にかからずに薬が欲しい人々の 確実な供給源となってきました

has provided a steady source for people wanting to self-medicate.

このような形での オピオイドの蔓延に対処するため

In response to this prescription epidemic,

処方は制限されるようになり オピオイドの路上取引は大幅に減少しました

people have been cut off, and this has greatly reduced the street supply

そして非意図的ながらも予測しえた結果が 薬物過剰摂取の蔓延です

The unintended but predictable consequence

確実に手に入る処方薬の供給に 頼り切っていた多くの人は

is an overdose epidemic.

ヘロインに走りました

Many people who were reliant on a steady supply of prescription drugs

その後 悲劇的にも非合法薬物の中心は

turned to heroin.

フェンタニルのような合成薬物に変わりました

And now the illegal drug market has tragically switched

to synthetic drugs, mainly fentanyl.

These new drugs are cheap, potent and extremely hard to dose.

他の種類の毒の場合を考えてみましょう

People are literally being poisoned.

Can you imagine if this was any other kind of poisoning epidemic?

What if thousands of people started dying

間違いなく緊急事態として扱うでしょう

from poisoned meat or baby formula or coffee?

そして直ちに安全な 代替物を提供するでしょう

We would be treating this as a true emergency.

また 法律を改正して

We would immediately be supplying safer alternatives.

犠牲者やその家族を支援するでしょう

There would be changes in legislation,

しかし 薬物過剰摂取の問題に対しては

and we would be supporting the victims and their families.

何もなされていません

But for the drug overdose epidemic,

それどころか私たちは薬物や その使用者を忌み嫌い続け

we have done none of that.

法的な取り締まりのために 資源をやみくもに費やしているのです

We continue to demonize the drugs and the people who use them

では これからどうするべきでしょうか？

and blindly pour even more resources into law enforcement.

So where should we go from here?

北アメリカ全土で 広く取り入れるべきです

First, we should fully embrace, fund and scale up

私はバンクーバーのような場所で ハーム・リダクションが

harm reduction programs across North America.

ケアと治療の命綱となってきた事を 知っています

I know that in places like Vancouver,

harm reduction has been a lifeline to care and treatment.
